Clinical Depression Survey for Bryony Kimmings

Thank you for participating in our survey.
I am Bryony Kimmings a performance artist from London. This summer I am making a new show with my partner Tim. The show is about mental health and men. More specifically living with Severe Clinical Depression.
The show will be a comedy. A way to expose the truths, our fears and lives spent dealing with the disease by allowing us to laugh instead of cry. Through humour it is possible to say the hardest things we have found. We are looking at the still existing taboos.
This is a survey to help us with our research, to see if the things we have experienced are universal and to gather stories. It is anonymous but material may be used in the show as stories and acts. So please only fill in if you are happy for this to happen.
This is for anyone who lives with the disease or knows someone close to them who does. Feel free to leave any of the questions blank and thank you in advance for your honesty and bravery.
